Biomedical Informatics - Student Reporting Forms

The following set of forms is part of the department's mechanism for tracking the progress of Post-doctoral fellows in the program.
There are five forms in all:

  1. a spreadsheet for keeping track of your courses and grades, including courses that you intend to take in the future
  2. a summary of the dates when you meet, or hope to meet, the various milestones associated with your degree.
  3. a research advisor's form (to be filled out by your research advisor).
  4. an academic advisor's form (to be filled out by your academic advisor).
  5. a student's reporting form (on which you tell us how you think you are doing and the ways in which the program could be more helpful to you).
This process will be required three times per year (all three semesters) and is not optional. Each semester we will ask you to fill out all five forms. You will be responsible for filling out #1, #2, and #5 first. #1 is an Excel spreadsheet that you should manipulate online. It will automatically calculate your GPA, etc. Postdocs should meet with their research advisors to get #3 filled out. Do this BEFORE you meet with your academic advisor. Then bring the entire set of forms with you to a meeting with your academic advisor. He or she will review forms 1, 2, 3, and 5, discuss them with you, and then fill out form 4. He or she will also sign off on forms 3 and 5. Note that you should sign forms 3, 4, and 5 as well. Once this is done, submit copies of forms each semester to Marina Honablue.
